COURSE OVERVIEW:

This course is designed to equip professionals with the knowledge, skills, and ethical considerations necessary to effectively coordinate services and supports for individuals with complex needs. This course will explore evidence-based approaches, models of case management practice, and the challenges faced by individuals, their families, and carers when accessing multiple services.

In Section A, participants will delve into evidence-based approaches and models of case management practice, providing a solid foundation for coordinating complex cases. They will explore the issues encountered by individuals and their families when navigating multiple services, including the impact of service duplication. Principles and practices of planning complex service inputs and working collaboratively with individuals and services will be emphasised, along with identifying indicators of self-harm and harm to others.

Participants will also learn about formal meeting processes, organisational standards, policies, procedures, legislation, and statutory mandates governing their practice. Responsibilities relating to duty of care, understanding family structure, dynamics, communication, and decision-making, as well as the requirements and boundaries of the coordination role, will also be covered to ensure participants are equipped to address the diverse needs of individuals with complex cases.

In Section B, participants will develop practical skills for working directly with individuals to determine and agree on service provision requirements. They will learn to develop comprehensive plans, obtain consent for information exchange, negotiate collaborative working arrangements, and implement duty of care responsibilities. Participants will also gain insights into establishing communication preferences, identifying and addressing barriers to outcomes, prioritising needs, facilitating meetings, minimising confusion and concerns, and facilitating communication between service providers to mitigate duplication.

Moreover, they will learn to monitor progress, obtain feedback, and implement further support as required, all while adhering to organisational policies and procedures for documentation.

Furthermore, participants will learn how to plan and coordinate resources, services, and supports effectively for individuals with complex needs, consolidating their understanding and skills acquired throughout the course.

By the end of this course, participants will be equipped with the knowledge, skills, and ethical awareness necessary to effectively coordinate complex case requirements, ensuring that individuals with complex needs receive comprehensive and coordinated support to achieve their goals and improve their quality of life.

Section B: Performance Tasks

·       Work with the person to determine and agree on the service provision requirements.

·       Obtain person’s consent to identify and exchange information with other services they are engaged with or being referred to.

·       Negotiate and agree on collaborative working arrangements and services to be provided.

·       Identify, implement and maintain duty of care responsibilities.

·       Provide information to the person about the coordination role.

·       Work with the person to establish their communication requirements and preferences.

·       Assess the person’s needs for interpreter and translation support and arrange access if required.

·       Work with the person and other services to identify barriers to attaining outcomes.

·       Work with the person to prioritise needs and communicate these with service providers.

·       Facilitate meetings to coordinate responsibilities and roles.

·       Work with other services to minimise person’s confusion and concerns in a coordinated manner.

·       Facilitate communication between service providers to identify and manage service duplication.

·       Work with the person and services to monitor progress toward the person’s goals.

·       Obtain the person’s feedback about services.

·       Identify and implement further support required to meet changing needs and progress towards the person’s goals.

·       Complete documentation in accordance with organisational policy and procedures.

·       Plan and coordinate resources, services and supports for three people with complex needs.
