INTRODUCTION:

Welcome to the Provide Person-Centred Care to People Living with Dementia course. This course only includes a challenge assessment designed to assess your knowledge and skills required to demonstrate competence in delivering person-centred, rights-based care that upholds the dignity, autonomy and wellbeing of people living with dementia in line with individualised plans and organisational requirements.

Providing person-centred care to people living with dementia involves preparing carefully for support by interpreting individualised plans, understanding dementia-specific needs and preferences, and working collaboratively with families, carers and others identified by the person to maintain a stable, familiar and supportive environment. It requires you to recognise when issues fall outside your role—particularly indicators of abuse, neglect or significant change—and to refer or report these in accordance with legislative, ethical and organisational requirements. The assessment will explore how you use supportive and respectful communication strategies tailored to the person’s abilities, needs, rights and preferences, and how you support engagement through verbal and non-verbal communication, reassurance and validation to relieve distress, confusion and agitation.

The course assessment focuses on your ability to implement meaningful, strengths-based daily support that reflects who the person is, rather than just their diagnosis. You will be required to demonstrate how you identify and use the person’s likes, dislikes, strengths, interests, reminiscences and routines to guide activities that are familiar, enjoyable and purposeful. You will also be assessed on how you encourage participation in activities that support ongoing independence, adjusting tasks to the person’s changing needs and preferences, and how you support the safe use of assistive technologies during activities to promote engagement, communication and independence. The assessment will consider how you communicate with families and carers to share care information, invite questions and maintain a consistent approach to support.

This assessment will also confirm your capacity to recognise and respond to behaviours as expressions of need, rather than “problems” to be managed in isolation. You will need to show how you use the individualised plan to recognise identified behaviours, potential triggers and early signs of stress or unmet needs, and how you develop and implement strengths-based strategies that reduce stressors and minimise adverse outcomes for the person, other residents or clients, and staff. This includes demonstrating that you can provide person-centred support to different people living with dementia by tailoring communication, activities and approaches to the unique abilities, history and preferences of each individual.

You will also be assessed on how you observe, record and communicate changes over time. This includes observing and recognising changes in behaviour, condition or responses to support, and promptly reporting these to a supervisor so that plans and strategies can be reviewed. You will be required to demonstrate how you complete, maintain and store reports and documentation about support, incidents and behaviour changes in line with legislative, ethical and organisational requirements, ensuring accuracy, objectivity, confidentiality and continuity of care.

By successfully completing this course assessment, you will have demonstrated that you can interpret and apply the core requirements of providing person-centred care to people living with dementia, including using individualised plans, working collaboratively with families and carers, applying respectful and effective communication, implementing meaningful and strengths-based activities, recognising and responding to behaviours and changes, and maintaining high standards of documentation—confirming your readiness to contribute to safe, dignified and individualised dementia care in your workplace.
