INTRODUCTION:
Welcome to the Determine & Address the Needs of People with Chronic Disease course. This course only includes a challenge assessment designed to assess your knowledge and skills required to demonstrate competence in recognising, understanding and responding to the complex and changing needs of people living with chronic disease, while working within your defined role and organisational requirements.
Determining and addressing the needs of people with chronic disease begins with understanding the broader context of chronic conditions, their impacts and common models of management. It requires the ability to identify a person’s chronic disease conditions and their likely impacts on daily life and wellbeing, and to gather accurate information on current treatments, inputs and supports from the person and other health professionals. You must be able to determine the level and type of support required within the scope of your own role, and adjust services and supports to the specific nature and progression of the person’s condition, recognising that needs may be physical, psychological, social and practical.
The course assessment focuses on your ability to work collaboratively with the person to support self-management and holistic wellbeing. You will be required to demonstrate how you involve the person in planning strategies to self-manage their chronic disease, explore social, emotional, physical, psychological and financial issues affecting their wellbeing, and provide information, advice and resources that build understanding and self-management capacity. The assessment will also examine your ability to identify unmet needs and make appropriate referrals to other services or professionals, work with family, carers and informal supports in ways that recognise their role and needs, and liaise with other agencies and professionals to support coordinated service delivery.
This assessment will also confirm your capacity to communicate clearly and work safely and accountably within a multidisciplinary support network. You will need to show how you meet communication and reporting responsibilities within the support network and your own role, respond to changes and variations in the person’s needs using a holistic, person-centred approach, and use effective and appropriate communication strategies with people living with chronic disease in diverse contexts and backgrounds. In addition, you will be expected to demonstrate how you review and refine support strategies over time based on feedback, outcomes and changing needs, ensuring that support remains relevant, respectful and tailored to the person’s situation.
By successfully completing this course assessment, you will have demonstrated that you can interpret and apply the core requirements of determining and addressing the needs of people with chronic disease, including understanding disease impacts and management, involving the person in self-management planning, working collaboratively with families and other services, communicating effectively, and continually reviewing and adapting support—confirming your readiness to contribute to safe, coordinated and person-centred care for people living with chronic conditions.
