INTRODUCTION:

Welcome to the Provide Individualised Services to Support Independence & Wellbeing course. This course only includes a challenge assessment designed to assess your knowledge and skills required to demonstrate competence in delivering individualised, person-centred support that promotes a person’s independence, choice and overall wellbeing in line with their goals and support arrangements.

Providing individualised services to support independence and wellbeing involves working in a person-centred and strengths-based way, recognising that each person’s social, cultural, spiritual, identity and sexuality differences shape how they wish to live and be supported. It requires you to understand and apply principles that uphold the person’s rights, dignity and preferences, while supporting access to services and resources in line with their individualised plan. This includes promoting and maintaining physical wellbeing and healthy lifestyle habits, identifying hazards and maintaining a safe environment, and responding appropriately to changes in the person’s physical condition.

The course assessment focuses on your ability to interpret and apply person-centred and strengths-based practice in real support situations, and to recognise the broad dimensions of wellbeing. You will be required to demonstrate how you support social, emotional and psychological wellbeing, including self-esteem and a sense of security, and how you encourage participation in social, cultural and spiritual activities and community networks. The assessment will also examine your understanding of mental health issues, risk and protective factors affecting wellbeing, and your ability to identify and respond to indicators of abuse, neglect and other risks to the person.

This assessment will confirm your understanding of the foundational concepts that guide safe and ethical support, including basic human needs, human development across the lifespan and how these influence support requirements at different stages of life. You will be required to show how you apply legal and ethical requirements in everyday practice, including duty of care, dignity of risk, human rights, privacy, discrimination, mandatory reporting and role boundaries. The assessment will also explore how you work within individualised plans, different service delivery models and relevant funding or service contexts, ensuring that your actions are consistent with the person’s goals and organisational expectations.

You will also be assessed on how you work effectively with others involved in the person’s life, including collaborating with families, carers and other key people while recognising the limits of your own role. This includes demonstrating how you monitor, report and document changes in a person’s independence and wellbeing in line with organisational policies and procedures, ensuring accurate, timely and confidential records that support quality and continuity of care.

By successfully completing this course assessment, you will have demonstrated that you can interpret and apply the core requirements of providing individualised services to support independence and wellbeing, including working in a person-centred and strengths-based way, recognising and respecting individual differences, promoting physical, social and emotional wellbeing, managing risks and legal obligations, and collaborating within the person’s support network—confirming your readiness to deliver safe, respectful and rights-based support in community and care settings.
